Brian is a 55-year-old taxi driver; he has just been discharged from hospital following a diagnosis of myocardial infarction. He had been diagnosed with angina 2 years ago after presenting to his GP with a 6-month history of mild chest pain and heaviness along with shortness of breath. He is due to start cardiac rehabilitation classes in 3 weeks’ time.
He lives with his wife Sue and his grown up children have now left home. Brian has smoked 20 cigarettes a day until his angina diagnosis and now uses a Vape device. He has always been overweight as is Sue, they have had several appointments with a dietician to help support their weight loss. Brian is also borderline type 2 diabetic. Brian is on various medication for his heart disease and diabetes which he doesn’t always take regularly, he says he doesn’t know what it is for. He has been encouraged by his cardiac nurse to be more active as his job is very sedentary.
Sue has expressed worry about his ongoing health and how this could impact them if he can’t work in the future. They don’t have any savings and are only eligible for the state pension.

• The physical changes brought on by this disease, summarising the signs and symptoms that may present.
• The treatment options available and the likely prognosis of the disease.
• The psychological impact on both the patient and his wife and carers/extended family.
• The support needs of the individual patient in this case, detailing how these support needs could be met.
